Apologies for not using 'search' for any of this, but ....
We (in the UK) have had a couple of 120 series (Prado) with 3.0 D-4D motors go bang recently with relatively low mileage. Some of the guys are understandably very concerned about the longevity of their expensive trucks. :? I'm looking for any similar incidents you may have experienced or be aware of?
There is a lot of stuff surfacing that seems to be related to problems with injector seals, and also problems with sludge in the sump blocking the oil pickup pipe, and the engine being starved of oil flow. You're happily driving along when suddenly the oil light comes on then the motor seizes 8O Bang - game over ...
I recall a while back that guys on the South African HiLux forum were unhappy with noisy injectors - there may be some form of relationship here, but that is tenuous at the moment. Same for the Injector rattle here on PP.
Some of the guys have now stripped out the injectors and replaced the copper washers with the new aluminium ones, and replaced o-rings at the same time. This solves the leaking seal problem. Toyota Ireland seem to be doing this for their clients free, Toyota UK won't admit a fault. Go figure ... :evil: Remember also that the UK got the D-4D back in 2002-ish so possibly the earlier faults were resolved by the time you got them in Aus?
At the same time there is evidence of carbon deposits in the intake manifold and there are other oil passages that need cleaning due to sludge build up. These are vehicles that are not abused and are serviced regularly!
I'm looking for any information of any similar issues in Sunny Oz on this - anecdotal or otherwise (although actual provable incidents would be best). We're not sure how we may want to use this, if at all, but we'd like to build up a bigger picture across users of these vehicle variants as close to world-wide as we can get. A similar post has appeared on LCCSA! It would be nice to know we're not alone in our concerns!

And for those that do their own servicing on D-4Ds, maybe drop the sump and clean the oil pickup next time :lol: A word of advice I've had is not to ever jack up against the sump (as if! :shock: ), and if you bash it be very quick to get it fixed - there is not a lot of room between the sump and the oil pickup.
